Support Co-ordination

Support co-ordination is a service offered by Assist Community Services as part of the National Disability Insurance Scheme (NDIS).

Support co-ordination helps NDIS participants to make the best use of the supports in their NDIS plan.

Support co-ordination is a capacity building support which helps participants to:

• Understand and use their NDIS plan to pursue their goals
• Connect participants with NDIS providers, community, mainstream and other government services
• Build confidence and skills to use and co-ordinate supports

Support co-ordination services offered by Assist Community Services recognises the diversity and choice of each individual, family and their carers.
