Assist Community Services promotes the implementation of person-centred approaches to planning and practice to support people with disability to develop and maintain their skills and independence, and to have a lifestyle based on their own interests and aspirations. All our supports are tailored to suit each participant, their needs and wants, this is done through the support pathway. At Assist Community Services we believe that person-centred supports are the most integral aspect in the delivery of supports to participants. Each participant of Assist Community Services develops a Support Pathway with the collaboration between staff and the participant, as well as the participants family or other stake holders depending on the participant’s wishes.
The Support Pathway details the supports, activities and resources that the participant determines will best support them in achieving and maintaining their desired outcomes. Appropriate information is sought and built into the plan to ensure the participant’s needs, support requirements, preferences, strengths and goals are included in the support pathway.
